Fill out the form below to enroll your child in the Chick-fil-A Capital Circle Kid's Birthday Club.  Your child will receive a special birthday party invitation in the mail inviting them to come and celebrate their birthday with the other club members who have a birthday that month. The party is on the first Friday of each month. Each birthday child will receive a free Kid's Meal (4 count), birthday cake, balloon and a special visit from our Chick-fil-a Cow.  Families of the birthday child are invited to attend, as well.  Children age 10 and under are eligible for enrollment.  We look forward to celebrating with you!
